This is what I put in a letter to a customer. Obviously you have to fill in the correct information for thepiano , but it gives you an idea of what i do.

Evaluation of Wurlitzer console, serial #: 162122, built in 1936
This is one of the first console pianos made by Wurlitzer. Although it might not be considered an “antique”, it certainly does qualify as an unusual instrument, and every effort should be made to preserve this piano.
Cabinet: Very good condition, 4 legs and outside lyre.
Soundboard: Good condition, some of the ribs are loose.
Keys: Ivory, in excellent shape, need to be cleaned.
Action: Nothing broken, but it is very dirty and needs work.
1. Shape hammers, 2. New damper felts. 3. Tighten screws. 4. Cleaning
5. Repin action centers. 6. Regulate. 7. Pitch raise and tune.
Rating scale based on 1 - 10 (10 being highest) Overall condition: 6
Pin Block: 6 Action: 4 Case: 7
Bridges: 5 Keys: (ivory) 7 Dampers: 3
Strings: 8 Soundboard: 7 Bench: ???
Appraised value of piano “as is”: $750.00
Estimated repair costs, as per above: Approximately $1300.00
Value after repairs: $2500.00
I estimate the value as listed for insurance or other purposes at the current replacement value, excluding Federal and other taxes. In making this appraisal, the Appraiser assumes no liability with respect to any action that may be taken on the basis of this appraisal.
